In the Dark Knight example, wouldn't it have been even better if it said "Bozo slides a grenade into the man's mouth" and then the next dialogue section had the character name "Bozo" instead of "Joker" in both instances? Seems like the real surprise comes in the paragraph where he pulls off the mask, and using "Joker" before that kinda ruins it.
