It rubs the lotion on it's skin. Totally freaked me out when the light hits the wall and we see the bloody fingernails. Great reveal and suspense. Catherine realizing she's not the first victim and will probably not live through this. Awesome screenwriting by creating suspense and giving us insight into Buffalo Bill's mind by using Hannibal Lector to Mentor Clarice.

The whole "It rubs the lotion on its skin" thing is straight out of the FBI behavioral science group, a finding they made about serial killers who will refer to their victims as "it" [3rd person] to dehumanize them, making it easier for them to victimize them.
