I read an interview or maybe it was on the BluRay that Brad Pitt actually checked himself in to a mental hospital for two weeks to understand how a manic person's behavior was and that he acted so well they almost didn't let him out. The visuals are not in the script (did Gilliam add them or the art director) but whoever did they add tremendous punch to this sequence, the tv cartoon head banging, the guy in the background humping the wall, and Brad Pitt's line to the nurse rapid fire "you're a very attractive woman" wasn't in script. I can't tell you how much I love this movie. Yes I'm also mentally divergent.

Much of the magic was Cole's (Bruce Willis) nonverbal reaction. The script conveys that Cole is engrossed with the TV; he also pretty clearly is wondering what's up with Jeffrey. Perhaps this part of Cole was foreshadowed in prior scenes, but to me it is a critical part of the scene not evident when reading the script.
